Job Summary:
Under direct supervision provides personal care and support to the residents of the Rehab, Assessment, Transition Unit.
Key Duties & Responsibilities:
1. Assisting patients with activities of daily living as outlined in the patient’s care plan such as:
– Personal hygiene.
– Dressing.
– Transfers.
– Toileting and managing continence.
– Feeding.
– Assisting with mobility.
2. Assists with managing patients behaviors as outlined in the care plan.
3. Assists with admissions, discharges, transfers.
4. Assists with maintaining a safe and orderly work environment. Reports any unsafe conditions/practices.
5. Participates as a member of the multidisciplinary team:
– Observes and informs supervisor/charge nurse of needs, problems and changes in residents condition.
– Assists co workers in providing patient care.
– Interacts with patients, families, visitors.
– Treats patients with respect and dignity.
6. Records accurately all pertinent information on patients health record following prescribed charting standards:
– Assists in updating Patient care plans
7. Maintains knowledge and practices safe use of equipment and efficient use of supplies
8. Time management:
– Keeps work personal and family life in proper perspective
– Exhibits flexibility by adjusting routines to changing circumstances and department/resident needs.
– Responds appropriately to urgent and difficult situations
– Rate of absenteeism does not negatively impact the day to day functioning of the unit.
9. Communication:
– Documents clearly and concisely.
– Demonstrates diplomacy and courtesy with regard to patients, staff, visitors, and others.
– Verbalizes in a clear, concise, direct, honest, and tactful manner.
– Shares information and provides feedback promptly.
– Respects and adheres to privacy and confidentiality of information associated with patients, staff and Alberta Health Services.
– Listens to what the others are saying, encouraging two way communication.
– Encourages free expression of opinion among staff.
– Communicates expectations clearly and realistically.
– Uses most effective method of communication for the situation.
10. Performs other related duties as required.

– Fast paced variable workload, physically/emotionally demanding.
– Shift work, moderate to heavy lifting, standing, walking, bending.
Qualifications:
Grade 12 diploma.
Successful completion of a P.C.A. program preferred.
Current BCLS-HCP certification preferred.
Experience in caring for the elderly preferred
Ability to work with people in a health care environment with professionalism and respect.
Ability to communicate effectively both verbally and in writing.
Proven interpersonal skills and the ability to get along with a wide variety of people.
Proven organizational, time management and sound decision making skills required.
Proven initiative and motivation required.
Ability to perform duties as outlined on the Physical Demand Statement (PDS) for the position.
Ability to safeguard AHS information as private and confidential.
Ability to follow and adhere to AHS policies, procedures, goals and objectives.
Ability to be a solid listener who goes out of their way to explain and resolve any concerns or problems presented within the department with the understanding that every individual has a different personality.

Experience with electronic charting preferred.
